You are a safe driver with a clean driving record You can obtain a clean Criminal Record Search with Vulnerable Sector screening You have up to date immunization records and can provide the following records of: MMR, DPT, Polio, Varicella, Hepatitis B, COVID-19 (2 doses), TB Skin Test (Step 1) *must be valid within last 12 month. The client has funding through the Choices in Supports for Independent Living program. She is wheelchair reliant, and needs help with standing pivot transfers in addition to exercise. Duties include (but may not be limited to) personal care, cooking, housekeeping, driving van with lift as well as shopping.
