MISSION The Complex Case Coordinator provides complex care coordination to vulnerable and chronically homeless youth within the shelter through assessment, planning, facilitation, care coordination, evaluation, and advocacy. The Complex Care Coordinator empowers youth, draws on their strengths and capabilities, and promotes an improved quality of life by facilitating timely access to the necessary supports that help reduce the risk of homelessness. The Pod Group is partnering with this organization to place a hands-on, highly resourceful, and dedicated individual to fulfill the role of a Complex Case Coordinator. PRIORITIES Provide complex care coordination through the development of a system of care for complex youth based on their unique needs. Provide case management with youth in developing and following through with comprehensive case plans designed to concentrate on obstacles preventing success in housing as well as promoting longer term stability and achievement. Manage individual progress of youth case plans and follow-up to ensure interim goals (daily, weekly) are still suitable and are being met. Conduct weekly in-person check-ins with active clients to ensure goals are being worked on based on the assessment. Collaborate with staff to help client’s secure affordable and appropriate housing using a Housing First for Youth model and to ensure wraparound services are being offered through outreach, mental health, harm reduction and housing. Accompany youth to viewings and meetings when necessary and act as an advocate/support person. Provide life skills teaching to help build client capacity to maintain housing, especially focusing on tenancy skills and knowledge. Conduct assessments, utilize care planning tools, facilitate care coordination, and advocate for options and services to meet youth’s comprehensive needs in order to achieve greater life stability Facilitate open communication with landlords/building superintendents to ensure positive relationships and resolve immediate concerns or issues. Form associations and relationships with a broad spectrum of community service agencies to be able to refer youth. Initiate contacts with or responds to contacts from a wide variety of individuals on matters concerning youth and their plan. Develop relationships with primary health/community health providers to ensure youth have access to health care and makes referrals to other professional supports depending on individual needs. Maintain confidential files on residents and ensures all appropriate documentation is completed. Other duties as assigned. TALENTS & EXPERTISE Bachelor’s Degree in Social Services or related field, or equivalent education (diploma) and experience combined with a registration/certification with a related accredited body (i.e., RSW, RSSW, RP, CYCP, etc.). 2-3 years experience with case management. Knowledge of housing first for youth, trauma-informed care, and harm reduction practices. Experience supporting youth experiencing chronic homelessness, mental illness and substance usage in obtaining housing, employment, educational and community opportunities. Ability to work rotating shifts including some weekends and holidays. A valid Ontario Driver’s License, access to a vehicle and $2,000,000 (minimum) liability car insurance coverage are mandatory. Valid CPI/ UMAB and First Aid/ CPR certifications. Satisfactory Vulnerable Sector Police Check. Must be flexible to work at Brampton and/or Mississauga location. Must be fully vaccinated against COVID-19, subject to applicable legislation. MISSION Reporting to the Director of Client Services and Wellness, the Clinical Manager will support youth by providing direct supervision to Mental Health, Addiction and Harm Reduction Services. The Clinical Manager is responsible for supporting staff with daily technical expertise and clinical assistance throughout the continuum of care, managing prevention, intervention, and after-care supports, with an emphasis on supportive counselling and case management. Using a client-centred approach and the Housing First for Youth philosophy, the incumbent will lead and inspire youth towards positive change. PRIORITIES Manage clinical and addiction services for youth seeking services and support. Supervise, coach, and support staff delivering mental health and harm reduction interventions with reinforcement from a strengths-based perspective, youth-driven care, and shared decision-making practices with youth. Provide technical training and clinical expertise to enhance staff competency and crisis intervention capacity in evidence-informed client assessment tools, risk mitigation strategies, safety planning, and counselling supports. Develop, enhance and implement policies and practices that align with the recovery model, developing competencies in mental health and addictions, evidence-based counselling methodologies (cognitive behavioural therapy (CBT), dialectical behaviour therapy (DBT)), motivational interviewing, strength-based counselling, solution-focused approaches, and just-in-time interventions in supporting youth. Support the referral and intake process, ensuring proper documentation and files are maintained and up to date for each youth. Build community connections and coordinate with service providers for further referrals to promote a continuum and holistic approach to care. Assist in increasing the availability and effectiveness of interventions for youth by using Continuous Quality Improvement (CQI) principles. Operationalize flexibility and access as the primary goals of interventions and continuously find ways to advocate, navigate, and eliminate barriers to rapid access to youth support and services related to their wellbeing, mental health, and addictions. Provide crisis stabilization and consultation for escalated complex cases with the coordination of wraparound support for youth in crisis. Participate on committees and task forces as required with other agencies addressing issues facing youth. Ensure that professional standards are maintained as related to client documentation and case management. Promote a low-barrier approach to youth support and facilitate access to services. Create a safe and secure setting for all youth accessing services at the emergency shelter and transitional program. Foster and model the importance of connection and belonging in building a positive and supportive work environment, using decolonized and anti-racist frameworks, to all staff. All other duties as required in the successful management of shelter operations while partnering with all stakeholders in support of organizational success.
