Persons) sector in Blanc Sablon, a warm and welcoming community located at the eastern end of the North Shore.\r\n\r\nMain responsibilities:\r\n Assess residents' psychosocial needs. \r\n Collaborate with the interdisciplinary team in planning and monitoring intervention plans. \r\n Support residents and their families in adapting to life in a CHSLD. \r\n Participate in coordinating community or external services as needed. \r\n Maintain clinical documentation in accordance with professional standards. \r\n \r\nRequirements:\r\n College diploma (DEC) in Social Work Technology or Intervention Techniques in Delinquency. \r\n Active membership in the OTSTCFQ (Order of Social Workers and Marital and Family Therapists of Quebec). \r\n Experience working with elderly clientele in an
